Root apex resection is a progressive method of treatment, which consists in removing the affected part of the root with the subsequent restoration of the function and appearance of the tooth. What is a Tooth Resection?

Resection is a surgical operation during which the infected part of the tooth root is cut off (from Latin reseco – truncate). Such an intervention is carried out under local anesthesia, therefore it is completely painless for the patient. During the resection, the doctor carries out repeated endotreatment of the canal with the replacement of the old restoration. A big plus of resection is the possibility of performing an operation even on a tooth that is under the crown – the surgeon gains access to the root through the bone tissue.

The main purpose of the resection is to remove the inflamed part of the root in order to keep it healthy and prevent further infection. Most often, resection of the anterior tooth or double-rooted teeth of the upper and lower jaw is performed.

Tooth root resection: basic indications

One of the indications for resection is an inflammatory focus, which is formed as a result of poor-quality canal filling. When treating pulpitis or preparing teeth for prosthetics, the canals are filled along the entire length. In places where the filling was not carried out thoroughly enough, infectious foci appear, the result of which is periodontitis, inflammation of the root apex, and the formation of granulomas.

Therapeutic treatment of a cyst is effective with a certain size of the formation – the larger the size of the inflammation, the more difficult it is to treat. Medicines are put in several times, which often leads to an exacerbation of the process. If the size of the inflammatory focus under the influence of drug endotherapy for 1 year remains unchanged (the size of the cyst does not decrease), in this case they resort to surgery (resection).

The main indication for resection is the absence of positive dynamics after 1 year of therapeutic endodontic treatment of the inflammation focus at the root apex.

Resection is not performed in the following cases:

  • 1
    obstruction of channels in healthy roots;
  • 2
    tooth mobility with periodontitis;
  • 3
    periodontal disease with significant exposure of the tooth neck;
  • 4
    chronic diseases (heart failure, neuroses, oncology, diabetes).

Root resection: how it works

The main stages of the procedure:

  • examination of teeth using X-ray equipment, which allows you to determine the size of the formation and clarify its position relative to the anatomical structures;
  • local anesthesia;
  • peeling of the gingival tissue flap;
  • cutting off the infected part of the root;
  • retrograde root canal filling;
  • suturing;
  • stitches are removed after a week.

The procedure is performed under local anesthesia. The doctor makes a gum incision through which the damaged root is cut off. The specialist gains access to the dental canal, the filling of which avoids re-infection. Retrograde filling of the root canal is performed from the side of the tooth root, which accelerates tissue regeneration. The incision is sutured.

After root resection for 3 months, excessive stress on the tooth should be avoided with food. An X-ray examination is necessary every 3 months in order to monitor recovery and prevent re-inflammation.

The only drawback in this procedure is the violation of the anatomical integrity of the tooth root, which somewhat reduces its functional characteristics. The effectiveness of treatment depends on strict adherence to the doctor’s recommendations and regular diagnostics. A competent rehabilitation plan will ensure a long service life of the tooth and no discomfort.

What should be done after surgery?

Recommendations that will be a good prevention of complications and speed up regeneration after the procedure:

  • after the operation, something cold should be applied to the cheek to reduce tissue swelling;
  • taking antibiotics prescribed by your doctor regularly;
  • avoid physical activity for a week;
  • strictly adhere to oral hygiene.
